Must be able to travel and provide case management services primarily in Georgia. Must be bilingual in Spanish and English
Flex schedule with non-traditional hours (includes evenings and weekends)

JOB PURPOSE: The Level 3 PRS Intensive Case Manager will provide short-term, trauma-sensitive treatment case management and supportive services to unaccompanied immigrant children released from federal custody. The Level 3 PRS Intensive Case Manager works in collaboration with program staff and supervisors to cultivate a client-centered and trauma-informed approach to service provisions.

Qualifications

ESSENTIAL JOB RESPONSIBILITIES:

  • Conduct weekly in-person home visits for the first 45-60-days of services

  • Monthly contacts for remainder of PRS to stabilize placement and assessing safety and appropriateness of child's placement with their caregiver (sponsor) and connecting child to services in the community to address risk factors.

  • Provide crisis intervention, safety planning, family preservation and additional therapeutic support.

  • Carry a maximum caseload of 3 Level 3 PRS cases.

  • Ongoing case management with a focus on screening and making appropriate referrals to address child's needs in the following domains: placement issues, safety issues,
    education, health, mental health, legal/immigration proceedings, substance abuse, independent living, criminal/juvenile justice, child enrichment activities.

  • Facilitate workshops, meetings, or conferences associated with the program, including, creation, management, and delivery of culturally centered instruction, and orientation materials to clients.

  • Actively establish, enhance and maintain liaison relationships with key government agencies, and non-profits/ community organizations.

  • Comply with policies and procedures governing the program and ensure that the program is operating at maximum efficiency.

  • Meet or exceed program productivity expectations on a regular and sustained basis

  • Other duties as assigned.

ESSENTIAL QUALIFICATIONS:

  • Experience with strength based, cognitive behavioral and trauma sensitive approach.

  • Minimum of 1 years of experience relevant to the position and working with vulnerable or migrant populations; 3 years preferred.

  • Proficiency in Spanish/English (written and spoken language skills)

  • Excellent verbal and written communication skills

  • Proficiency in Microsoft Office applications

  • Able and willing to travel domestically up to 60% of time.

  • Must pass FBI and CA/N checks

  • Must be 21 years of age

  • Driver's License with clean driving record required.

EDUCATION: Master's degree in social work, psychology, sociology, or other relevant behavioral science in which direct clinical experience is a program requirement; or a Bachelor's degree with at least five years of clinical experience. Licensed or eligible for licensure.

ATTENDANCE: Must maintain regular and acceptable attendance at such level as is determined in the employer's sole discretion.

LICENSES: Driver's License with clear record.

VEHICLE: Must have daily use of a vehicle without prior notice.

OTHER: Must be available and willing to travel to various locations and with such frequency as the business need dictates.
