Responsible to coordinate intake of patients new to NHCI, to assess patients'clinical needs, determine placement along continuum of cancer care, identify barriers to care, and coordinate referrals to appropriate NHCI providers. Includes scheduling of initial appointments, coordinating collection and transfer of medical records, and facilitation of referrals to additional NHCI clinical and support resources based on barriers/needs identified.

Support and facilitation of services for patients may be provided at any phase of care, from pre-diagnosis through treatment, survivorship or palliative care/end of life, and may be at patient or referring provider request to establish care at NHCI or to obtain a second opinion regarding treatment options.

Requires solid knowledge of general oncology and recent experience in caring for oncology patients. Understanding of cancer diagnosis, staging and treatment is required. Must develop and maintain working knowledge of current geographic overview of NHCI oncology providers by specialty and resources available throughout the NHCI market footprint. Strong customer service skills and an ability to work with providers and all levels of clinic personnel throughout NHCI is essential.

Required

  1. Active Professional Registered Nursing licensure in the state of Georgia.
  2. Bachelor of Science Degree in Nursing from an accredited university and two years of recent direct oncology nursing care experience OR Associate Degree in Nursing with three years of recent direct oncology nursing care experience.
  3. Working knowledge of Microsoft Office Products Outlook, Word, Excel, and Power Point.
  4. Maintains oncology nursing certification or completes at least 36 hours of oncology-specific continuing education in a 3-year period.
  5. Strong verbal and written communication skills; ability to work well with all levels of clinic personnel.
  6. Strong analytical and critical thinking skills applying oncology nursing education, knowledge and experience.
  7. Strong customer service skills
  8. Ability to take initiative and work independently with minimal supervision.
  9. Attention to detail

Preffered:

  1. Previous experience working with culturally diverse populations.
  2. Strong working knowledge of healthcare systems and community resources.
  3. Oncology Nursing certification.
