JOB SUMMARY:
The Licensed Behavioral Health Therapist is responsible for providing outpatient care and coordinating services across the care continuum for defined patient populations. This includes risk assessment, transitional care planning, psychosocial and functional status assessment, patient advocacy, motivational interviewing, education of the patient/family and monitoring quality indicators that demonstrate outcomes from service provided. The Behavioral Health Therapist position works collaboratively with the primary care team to meet patient's needs by assisting patients to safely and effectively navigate through the care continuum.
The Behavioral Health Therapist performs concurrent care review in accordance with approved clinic and quality guidelines; performing post clinic and post hospitalization follow-up, providing care gap management, consulting and collaborating with patients, providers and other support team resources as appropriate.
JOB QUALIFICATIONS:
  • Master's degree is required.
  • Experience in motivational interviewing /health coaching / goal setting is required.
  • Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW), Certified or Licensed Clinical Professional (CCM Certified Care Manager), or applicable certification and licensure is required.
  • All licensure must be must be current and active for the state of Georgia.
  • Must have current BLS certification from American Heart Association.
