Coordinates and supervises diabetes education activities for inpatients, outpatients, and individuals and groups of patients with diabetes ranging in age from adolescence to adulthood. Coordinates the diabetes education plan with other team members, and acts as liaison to the primary nurse and physician. Serves as role model and resource to staff, and facilitates Quality Improvement activities. Must be willing to work flexible hours, including weekends, evenings, and weekend call. Must complete a minimum of twenty-four (24) hours of continuing education in diabetes and education principles during initiation on job, and must complete six (6) hours of diabetes education or continuing education each year.

REQUIRED:
1. Bachelor's degree in Nursing, or Bachelor of Science in Nutrition, plus one (1) year experience in patient education, or diabetes patient care. 2. Certified Diabetes Educator (CDE). 3. Good written and oral communication skills. 4. Ability to work effectively with a multidisciplinary team. 5. Demonstrated knowledge of adult education principles.
PREFERRED:
1. Certified Diabetes Educator (CDE) certification.
